A well-established domiciliary care provider in Taunton is looking for an experienced Care Coordinator to join their team.

This is a key operational role — you’ll be responsible for scheduling care visits, managing the care worker rota, and ensuring all clients receive their care on time.

You’ll be responsible for:
• Building and maintaining care worker rotas to ensure all visits are covered
• Responding to last-minute changes, sickness, and emergency cover requirements
• Acting as the first point of contact for care workers and clients
• Conducting initial client assessments and care plan reviews
• Maintaining accurate records on the care management system
• Liaising with social workers, GPs, and families

Requirements:
• Previous care coordination or senior care worker experience
• Strong organisational and communication skills
• Calm and decisive under pressure
• Enhanced DBS
• Full UK driving licence
