We’re recruiting Community Health Care Support Workers across Somerset to support NHS and community health teams.

You’ll visit patients in their own homes and in community settings, supporting clinical staff to deliver care closer to home.

You’ll be:
• Supporting patients with personal care, mobility, and daily living
• Recording observations and reporting changes to the clinical team
• Assisting with wound care, catheter care, and other clinical tasks (training provided)
• Building trusting relationships with patients and their families
• Working autonomously and managing your own caseload effectively

Requirements:
• Previous care or healthcare experience
• Full UK driving licence and own vehicle (mileage paid)
• Enhanced DBS
• Caring, professional, and reliable
